Reporting an Incident

Overview

You can report work place incidents through the Reporting an Incident function.

The Affected Details Section

Employee Effected

  1. Click on the Safety menu.
  2. Click on the Report Incidents submenu.
  3. Select the employee option from the Who did this occur too? field.
  4. Select the employee's name from the Employee field.
    Important: The only employees shown in the drop-down are the ones who directly report to the user raising the incident.

Third Party Effected

  1. Click on the Safety menu.
  2. Click on the Report Incidents submenu.
  3. Select the Third Party option from the Who did this occur too? field.
  4. Complete the following fields:
    • Name.
    • Date of birth.
    • Contact number.
    • Position.
    • Address.
    • Manager/supervisor.

The Incident Details Section

  1. Click on the Safety menu.
  2. Click on the Report Incidents submenu.
  3. Complete the following fields:
    • Incident type.
    • Date of incident.
    • Time of incident.
    • Incident address.
    • State.
    • Location.
    • Incident description.
    • Injured body parts.
    • Injury description.

The Witness Section

Adding a Witness

  1. Click on the Safety menu.
  2. Click on the Report Incidents submenu.
  3. Click the Add a Witness button.

  4. Enter the following information:
    • Name.
    • Contact details.

Deleting a Witness

  1. Click on the Safety menu.
  2. Click on the Report Incidents submenu.
  3. Next to the nominated witness, click the Minus button.

The Treatment Section

Treatment Provided

  1. Click on the Safety menu.
  2. Click on the Report Incidents submenu.
  3. Under the Was treatment provided? question, click the Yes button.

  4. Complete the following fields:
    • Place taken for treatment.
    • Treatment provided.

Treatment Not Provided

  1. Click on the Safety menu.
  2. Click on the Report Incidents submenu.
  3. Under the Was treatment provided? question, click the No button.

File Upload Section

Important: The maximum uploaded file size allowed is 2MB.
  1. Click on the Safety menu.
  2. Click on the Report Incidents submenu.
  3. Click the Upload button.

  4. Click the Submit button.
    Important: The nominated Worksafe Authority will receive an email once the user submits the incident.
