You can report work place incidents through the Reporting an Incident function.

Reporting an Incident

  1. Click on the Safety menu.
  2. Click on the Report Incidents submenu.

The Affected Details Section: Employee Effected

  1. Select the employee option from the Who did this occur too? field.
  2. Select the employee's name from the Employee field.

The Affected Details Section: Third Party Effected

  1. Select the third party option from the Who did this occur too? field.
  2. Complete the following fields:
    • Name.
    • Date of Birth.
    • Contact Number.
    • Position.
    • Address.
    • Manager/Supervisor.

The Incident Details Section

  1. Complete the following fields:
    • Incident Type.
    • Date of Incident.
    • Time of Incident.
    • Incident Address.
    • State.
    • Location.
    • Incident Description.
    • Injured Body Parts.
    • Injury Description.

The Witness Section: Adding a Witness

  1. Click the Add a Witness button.

  2. Enter the following information:
    • Name.
    • Contact Details.

The Witness Section: Deleting a Witness

  1. Next to the nominated witness, click the Minus button.

 The Treatment Section: Treatment Provided

  1. Under the Was treatment provided? question, click the Yes button.

  2. Complete the following fields:
    • Place Taken for Treatment.
    • Treatment Provided.

The Treatment Section: Treatment not Provided

  1. Under the Was treatment provided? question, click the No button.

File Upload Section

  1. Click the Upload button.

  2. Click the Submit button.
    Important: The nominated Worksafe Authority will receive an email once someone has submitted the incident.
