Mobile: Reporting an incident

Overview

As an employee, you could have a safety incident in your workplace that can range from a near miss right through to more serious incidents such as electrocution. These incidents need documenting when they occur to ensure your organisation can track, manage and report on the health and safety of both their employees and there preventive safety procedures.

The Reporting an Incident on the mobile app allows you to submit a safety incident report for yourself, another employee or a contractor. You can also use this form to specify any first aid given, if there were any witnesses, and attach any supporting documentation.

Getting started

Reporting an incident: Employee
  1. Log into the mobile application.
  2. Tap the Plus button.

  3. Tap the Safety Incident button.

  4. Tap on the Who Did This Occur To?
    button and select the employee option.
  5. Tap on the Employee button and
    select the relevant employee.
  6. Tap the Continue button.

  7. Complete the following fields:
    • Incident type.
    • Date of incident.
    • Time of incident.
    • Incident address.
    • State.
    • Location.
    • Incident description.
    • Injured body part.
    • Injury description.
  8. Tap the Continue button.

  9. Tap the Add New button to
    add any witnesses.

  10. There are now two treatment options:
    • Yes: Tap the Yes button and
      complete the following fields:
      • Place taken for treatment.
      • Treatment provided.
    • No: Tap the No button.
  11. Tap the Plus button to add
    any attachments.
  12. Click the Submit button.
Reporting an incident: Third party
  1. Log into the mobile application.
  2. Tap the Plus button.

  4. Tap on the Who Did This Occur To?
    button and select the third party option.
  5. Complete the following fields:
    • Name
    • Date of birth.
    • Contact number position.
    • Address.
    • Manager / Supervisor.
  6. Tap the Continue button.

  7. Complete the following fields:
    • Incident type.
    • Date of incident.
    • Time of incident.
    • Incident address.
    • State.
    • Location.
    • Incident description.
    • Injured body part.
    • Injury description.
  8. Tap the Continue button.

    add any witnesses.
