The Medical Disclosure Statement allows for employers to determine whether an employee is able to perform the inherent requirements of their role.
The Medical Disclosure Statement can be found in the employees' employee file if the organisation has enabled that tab.
Employees who claim to suffer from pre-existing injury, illness, medical condition or allergy are required to provide the following details:
- Whether they have made a claim for workers' compensation before
- Nature on injury/medical condition
- Whether they have a medical clearance to perform their pre-injury duties
- Current work restrictions
The employee's manager, or organisation's owner in instances an employee does not have a manager, will be notified if the employee has made the above claim through a scheduled task.
The scheduled task provides a link to access the employee's Medical Disclosure Statement, where the manager or owner is required to acknowledge the employee's Medical Disclosure Statement.
